I know the Cairns PHOs/regs log all their overtime and it gets paid without drama.
It'd be somewhat familiar with some colleagues working there, and I've worked briefly in Cairns, but I've heard mixed things about the FNQ training network.
Toowoomba and Cairns come to mind as well rounded departments that see a wide range of presentations (including trauma) and (imo) have good teaching and supportive consultants.
Cairns hospital is fantastic for learning and culture.
If you find a place with a supportive culture (two with good recommendations and reputations are Cairns and Toowoomba) then it is possible.

I’m PGY3 and have worked in Cairns and it’s satellite rural hospitals for all that time.
Cairns and Townsville have excellent training and would see your regional experience as a plus… as would Darwin in the NT which asl has a training program with both BPTs and AT rotating to Katherine for real authentic experience.
Cairns is regional but high acuity because it's isolated with a high population, it's got a fantastic staff mix and strong culture.

I'm from SA so my knowledge of Queensland health is a little limited, but I've heard that Cairns and Townsville offer great learning opportunities and are generally friendly places to work and live.

I’m interested in working in Cairns hospital but would love to hear experiences (good/ bad) of what it’s like for training, culture, rostering, leave, etc.
I worked for a few months in Cairns as an RMO then got bumped up to a PHO (principal house officer I think) which is basically a reg not in training who gets paid reg rates.
Depends on the hospital but CHHHS Interns don’t get a lot of overtime when going rural - meaning the hospitals send you home on time.
